Every US state allows some form of it, with limits that vary by state. Updated Nov 5, 2026. APTA tracks the rules. As of July 1, 2025, all 50 states, the District of Columbia and the US Virgin Islands allow direct access in either unrestricted or provisional form, and some states still limit treatment without a referral. Medicare doesn't require a referral to start, but it pays only when a physician or nonphysician practitioner certifies the plan of care, and the plan is recertified at least every 90 days or sooner if it changes significantly. Private insurers set their own rules. Aetna, for example, stopped requiring a referral or signed plan of care for physical therapy in June 2025. Check each payer before you advertise that no referral is needed. Patients ask AI assistants whether they can book a physical therapist without a referral. A page that answers that for your state and the plans you take gives them a clear answer to repeat.
